As an Agency Provider, We appreciate how important it is to our friends in the Disabled Community to participate in the workforce, day centers, and other important community activities. We also, understand the importance of support and encouragement in helping friends reach stated outcomes listed in the Individual Service Plan.

Support includes advocating for the rights of the individuals, assistance with ADL's, serving as a trusted intermediary, helping to effectively communicate with the landlord, doctor, bank, and other necessary social agencies.

• Choosing a healthier diet

• Self-care practices

• Scheduling & facilitating medical appointments

• Medical Administration

• Locating and moving into a new apartment

• Securing or replacing state ID & social security card

• Setting up an auto payment system for recurring bills

• Filing taxes

• Providing daily companionship

• Meal preparation

• Planning community activities & travel
