Job Description
All descriptions and duties are subject to change at the sole discretion of Collective Care management.

Duties include but are not limited to:

  • Timely arrival to work: Medicaid Services consumed by individuals in the DD Community can be time-sensitive. In addition to this, the Individuals function best with consistency. Individuals depend on their staff to promptly help them accomplish daily activities that are important to them. Arriving at their workplace on time, receiving medication, and being greeted upon their return home from their Day Center all require prompt attendance from the staff.

Unless otherwise noted, homes receiving HPC services from Collective Care are staffed during 3 shifts: 

           1st.  6 AM - 2 PM
2nd. 2 PM - 10 PM
3rd. 10 PM - 6 AM

Current availability : 2 PM - 8 PM (M-F)


  • Daily documentation: Collective Care personnel must complete service documentation for each shift and per individual served. All entries must comply with the rules established by the State of Ohio, Medicaid, and The Board of Developmental Disabilities. Duties are to be administered with integrity, respecting the rights and best interest of the individuals.

 
  • Ensure a safe and secure environment for Individuals: While on duty, direct service providers must limit their preoccupation with personal cell phone use, television, napping, and other non-work-related distractions. Steps to safety:

  • Handling and securing sharp objects in the home, 

  • Monitoring the use of the stove and other fire hazards, 

  • Securing the home (doors & windows) at the appropriate times, 

  • Attentive to the individual’s location and activities at home and in the community, 

  • Proper food prep and consumption, 

  • Safe transportation including seatbelts and traffic laws,

  • Keeping the home and vehicles sanitary (house cleaning, personal hygiene, etc.) 

  • Conflict resolution: de-escalation and preventative measures,

  • Appropriate Medication Administration

  • Meal Preparation: Ensure that meals are prepared promptly and according to an approved menu. Follow all dietary restrictions and guidelines per individual.

  • House cleaning: Complete the list of daily cleaning tasks assigned to the staff. Ensure that clients fulfill their assigned daily household chores.

  • Transporting Clients: DSPs transport clients to work, appointments, shopping, and community outings. Overnight staff may transport for emergencies, such as evacuations and hospital visits.

  • Monitor Non-Staff visitor(s): This is a private residence for the individuals. All visitors should be invited guests of the individual living in the home. Staff are not allowed personal visits in the individual's home while on duty.

  • Conflict Resolution: Appropriately mediate client disagreements. Prioritize safety while respecting each individual’s voice and viewpoint. Treat the individuals as you would want to be treated, with dignity and respect. 

  • Filing Incident Reports: DSPs are mandated to report all incidents affecting the safety and well-being of Individuals receiving services. DSPs must follow established protocol concerning Unusual and Major Unusual Incidents (UI/MUI).

  • Advocate for the best interest of the Individual: Collective Care’s motto is “Helping Friends Live Their Best Lives”. DSPs must always take a person-centered approach when providing services, asking themselves “how would I feel if I were in their situation, what would be my desired outcome?”
